Care Management at Peach State Health Plan 

 

Managing your health can sometimes feel like a lot. You don’t have to do it alone. Peach State Health Plan’s Care Management Team can help make it easier to get the care, services and support you need.

Care management is when someone helps you stay healthy and get the care you need. A care manager works with you to understand your needs and health goals. Together, you can create a personalized care plan that works for you. Care management may be helpful if you have an ongoing health condition, have complex health needs, or need extra support after a hospital stay.

How a Care Manager Can Help

Depending on your needs, your care manager can help you:

  • Find doctors and other health care providers.
  • Understand and manage your health conditions.
  • Keep track of your prescription medicines.
  • Connect with resources and services in your community.
  • Get supplies or equipment you may need at home.
  • Set up visits with mental health professionals, such as therapists.
  • Connect with programs that support your health and other needs.
  • Find support for alcohol or drug use.
  • Create a plan to help you get well and stay well.
  • And more.

Your care manager can also help you understand your care and connect the different people and services involved in supporting your health.

How Do I Get Care Management?

Care management is part of your Peach State Health Plan benefits and is available to members at no cost.

There are several ways to get started:

  • Your doctor, health care provider or another service provider can refer you.
  • You can refer yourself by calling our Care Management team at 1-800-504-8573 (TTY/TDD: 1-800-255-0056).
  • A parent, caregiver, family member or friend can call our Care Management team to refer a Peach State Health Plan member.

After you enroll, a care manager will contact you to learn more about your health, needs and goals. They will work with you to create a personalized care plan and stay connected to provide support along the way.
