Mom’s Meals® Home-Delivered Meals Program

Important: Some rules apply. You need to be a member of Peach State  Health Plan (PSHP) to be eligible for Value Added Benefits (VABs). PSHP doesn’t cover reimbursement fees for VABs. Processing may take up to  30 business days.

PSHP offers a home-delivery meal program for eligible members, enrolled in:

  • Case Management (CM), Disease Management (DM),and/or Care Coordination (CC) participants or those that would benefit from participation in CM, DM or CC.
  • Prenatal program (must be receiving CM or CC support). 
  • Chronic Conditions (such as Diabetes and Hypertension)
  • Behavioral Health 
  • Post Discharge
  • Social Determinants of Health

Qualification Requirement: Must be enrolled in Case Management, Disease Management, and/or Behavioral Health program.

Program Includes:

  • Two meals a day for four weeks
  • Optional nutritional counseling
  • Customized meals tailored to each member’s condition(s)
  • Delivery to doorstep within five business days
