Gym Membership

This benefit pays for a gym membership for members aged 18 and up. Teens aged 13-17 can also join the Active & Fit Teen program.

Requirements:

You need to be an active PSHP member.
Eligible members ages 13-17 must complete a wellness visit plus any two of the following vaccinations:
- Tdap or DTaP (Tetanus, Diphtheria, & Pertussis)
- Meningococcal
- HPV
- FLU (Influenza)
Adult Gym Benefit:
- Annual well visit (age 18+) with two of the adult immunizations:
  - Tdap or DTaP (Tetanus, Diphtheria, & Pertussis)
  - Meningococcal
  - COVID
  - FLU (Influenza)

Qualifying health activities must be completed within the last 12 months or at approved timeframes.
