Health and Wellness

Important: Some rules apply. You need to be a member of Peach State Health Plan (PSHP) to be eligible for Value Added Benefits (VABs). PSHP doesn’t cover reimbursement fees for VABs. Processing may take up to 30 business days.

At Peach State Health Plan (PSHP), we want you to stay healthy and  happy. That’s why we offer special benefits to help you do just that.  Here's how you can get them:

To join the YMCA® Family Membership or Gym Membership, fill out the form below. If you’re under 18, ask your parent or guardian to fill it out for you. You’ll also need to do some healthy activities. We’ll let you know if you don’t meet the requirements.

YMCA® Family Membership

This benefit gives you a family membership at certain YMCAs. It’s for kids aged 6-18 and their immediate family (up to six members).

Requirements:

  • Someone in your family must be and active PSHP member aged 6-18. 
  • Get a Sports Physical (ages 6-18), including the HPV vaccine (ages 9-13).
  • Have a Weight assessment and counseling for nutrition and physical activity (WCC).
  • Show the voucher at a YMCA between July 1, 2024, and June 30,  2025. 
  • Quality health activities must be completed after July 1st, 2023.

Gym Membership

This benefit pays for a gym membership for members aged 18 and up.  Teens aged 13-17 can also join with the Active & Fit Teen program.

Requirements:

  • You need to be an active PSHP member.
  • Teen Program: Get a Sports Physical (ages 6-18), including the HPV vaccine (age 13).
  • Teen Program: Have a Weight assessment and counseling for nutrition and physical activity (WCC). 
  • Get an Adult well visit (age 18+) with one of the adult immunizations.
  • Quality health activities must be completed after July 1st, 2023.

How to Get Your Benefits: Registration starts on July 1, 2024. If you're under 18, your parent or guardian must fill out the form.

Note: Swimming lessons aren't included in the YMCA Family Membership. You can apply for them under the Healthy and Active Children program. Summer camps are not part of the Health and Wellness program. To sign up, you need a Medicaid ID. If you don't have it, call Member Services  at 1-800-704-1484.
