Qualification Requirements:

Qualifying health activities must be completed within the last 12 months or at approved timeframes.

Age 21-49 (Female): Complete Cervical Cancer Screening, plus, wellness exam and Flu (Influenza) vaccine.
Age 50-75 (Female): Complete Breast Cancer Screening, plus, wellness exam and Flu (Influenza) vaccine.
Age 21-44 (Male): Complete Annual Wellness Visit, plus, Flu (Influenza) vaccine
Age 45-74 (Male): Complete Colorectal Cancer Screening, plus, wellness exam and Flu (Influenza) vaccine

One membership per calendar year
Members must present the gift card within the current benefit year (July 1, 2026, through June 30, 2027)
