Care That Connects: A Member’s Healing Journey with Peach State Health Plan

Recovering after surgery can be challenging, especially when the care and supplies needed to heal safely at home are delayed or unavailable.
After a recent surgery, one Peach State Health Plan member required specialized home health services, medical supplies, and feeding equipment to support recovery. While some equipment arrived initially, follow‑up care and ongoing supplies ordered by the provider were delayed. Without the right support at home, the member experienced complications that led to multiple hospital readmissions and significant stress, including paying out of pocket for supplies.
When Peach State Health Plan’s Care Management team became aware of the situation, Care Manager Kimberly Jackson acted quickly to help coordinate support.
Kimberly worked closely with the member, the care team, and provider to identify gaps and understand the challenges the member faced at home. Through ongoing conversations, she uncovered key barriers, including difficulty managing wound care and the financial strain of purchasing supplies independently.
Determined to help, Kimberly researched available options and coordinated solutions. She successfully connected the member with a home health provider and a medical supplier that could deliver the appropriate feeding equipment and essential dressing supplies.
