POPULATION HEALTH PROGRAMS

Peach State Health Plan offers highly personalized and patient-centered programs that are designed to assist members and their family support systems with managing their medical conditions and leading to the road of becoming healthy. You must be enrolled in Case Management, Disease Management, Behavioral Health, and receiving Care Coordination services.

Eligibility Statement: Eligibility requirements apply. The member must meet criteria.

The following program is available for:

  • Mom’s Meals® home delivery program available
  • Gym Membership at participating gyms for ages 16 & up
  • Hypoallergenic Bedding (annually) for members with asthma

Mom’s Meals® home-delivered meals program

Peach State offers a home-delivered meals program for qualified members. Including members enrolled in:

  • Pre-natal program
  • Chronic Conditions i.e (Diabetes and Hypertension)
  • Behavioral Health
  • Post Discharge
  • Social Determinants of Health

Qualification Requirement:

Must be enrolled & active in a Case Management/Disease Management/Behavioral Health program.

Program includes:

  • Standard program provides 2 meals/day for 4 weeks
  • Nutritional counseling
  • Customized to each member’s condition(s)
  • Delivery to doorstep in 1-5 business days

Gym Membership (ASH Fitness)

For enrolled Population Health/or Case Management members; PSHP will pay the annual membership and monthly fee at participating gym age 18 and older. 

ASH FITNESS  Teen program: ages 13-17

Select to either go to a gym, or utilize the at-home gym and videos.

Requires approval from Care Managers

Hypoallergenic Bedding

Qualified Population Health members may receive hypoallergenic bedding with mattress and pillow casings (may control asthma triggers).

Requires Care Manager approval

Qualification Requirement:

  • Active Peach State members with a diagnosis of asthma.
  • Limited to a maximum of $100.00 annually.
