Family Resources

The Care Coordination team maintains family resources with healthy family connections in mind. At Peach State Health Plan, we offer a variety of family-based services and resources to assist family in their time of need.

Eligibility Statement: Eligibility requirements apply. These programs are available while funds are available. The eligible must meet criteria.

Care Coordination services are available based on specific criteria:

  • Childcare Assistance: Up to $175 annually to help pay for childcare costs
  • Housing/Utility Assistance: $250 annually for rent deposit, rental fees or utility assistance.
  • $50 gas card for eligible members ages 21 & up

Childcare Assistance

Members can receive up to $175.00 for child care assistance. This could be used for up to 4-weeks annually to help pay childcare costs for school age children.

Qualification Requirements:

  • All active Peach State members ages 18 or over.
  • Must meet emergency childcare assistance criteria (job interview, DFCS, Dr., BH Appointments, Housing Authority)
  • Service must be for member or dependent with active eligibility.
  • Member may use the benefit once per year. Will pay for 0-4 early education and also school ages/school break care (typically children up to ages 13).

How to Access Benefit:

Members should contact The Customer Service Dept. at 1-800-704-1484 to request to this benefit.

 

Housing/Utility Assistance

Eligible members can receive up to $250 annually. The Home Allowance benefit can be used for rent assistance with rental deposit or application fee or utilities.

Qualification Requirements:

  • All active Peach State members ages 21 and older.
  • Member may use the benefit once per year.
  • Must have exhausted all available resources through Shared Services.

How to Access Benefit:

$50 Gas Card

Provides eligible head of household with an emergency $50 gas card, (1x time annually) 

  • Member may use the benefit once per year; one gas card per household. 
  • All active Peach State members ages 21 and older. 

Must meet emergency transportation assistance criteria: 

  • Service must be for member or dependent with active eligibility. Requires approval from Care Managers or Care Coordination Team
